    Habit Reversal Training

    Habit Reversal Training (HRT) is a behavioral treatment for repetitive behaviors like hair pulling (trichotillomania), skin picking, nail biting, tics, and other body-focused repetitive behaviors. It works by building awareness of the behavior and replacing it with a competing response.

    Developed by Nathan Azrin and R. Gregory Nunn, HRT is one of the most effective treatments for these behaviors. The core principle is that you can't do two physically incompatible behaviors at once.

    The Three Main Components

    1. Awareness Training

    •Describe the behavior in detail
    •Identify warning signs (urges, triggers)
    •Notice the sequence of movements
    •Catch the behavior earlier and earlier
    •Practice in a mirror if helpful

    2. Competing Response Training

    •Choose a behavior physically incompatible with the habit
    •Something you can do inconspicuously
    •Hold for 1-3 minutes or until urge passes

    Examples:

    •Hair pulling → Clench fists or grip object
    •Nail biting → Put hands in pockets or under thighs
    •Skin picking → Make a fist or hold something
    •Tics → Tense the involved muscles isometrically

    3. Social Support

    •Tell someone about your plan
    •Ask for gentle reminders if they notice the behavior
    •Celebrate successes together

    Additional Strategies

    Stimulus Control:

    •Modify environment to reduce triggers
    •Wear bandaids on fingertips
    •Use fidget toys as alternatives
    •Cover mirrors if that's a trigger location

    Relaxation:

    •Tension often triggers the behavior
    •Practice relaxation techniques
    •Address underlying stress

    Self-Monitoring:

    •Track frequency, triggers, context
    •Notice patterns
    •Celebrate reductions

    When to Seek Help:

    •If behaviors are severe or distressing
    •If self-help isn't working
    •Look for therapists specializing in HRT or BFRBs

    Why habit reversal works:

    •You can't do two opposite physical actions simultaneously
    •Awareness is the first step—you can't change what you don't notice
    •Competing response gives you something TO do, not just "don't do it"
    •With practice, the urge weakens (habituation)
    •Success builds confidence and motivation
